Derrick is loved and remembered by His parents: Ron and Donna Poliakiwski of Mannville; one brother: Kory (Angel) Poliakiwski of Airdrie; one sister: Rhonda (Devon) Whitten of Edmonton, two nieces: Cassidy and Autumn Poliakiwski, one nephew: Dawson Poliakiwski; grandmothers: Jean Poliakiwski of Mannville and Dorothy Mergl of Vermilion as well as numerous aunts, uncles, cousins and special friend: Anne Bardoel. He was predeceased by his grandfathers, Nick Poliakiwski and Frank Mergl.
