Benjamin Kozak Obituary

On Tuesday, June 9, 2009 Mr. Ben Kozak of Wainwright, Alberta passed away at the Wainwright Auxiliary Hospital at the age of 75 years. He is survived by his loving family; Helen Kozak; two children, Bruce (Tracie) Kozak, Tzea (Elvin) Sawchuk all of Wainwright; four grandchildren, Brayden (Erin Blanchard) Kozak of Edmonton, Alberta, Desirae Kozak of Edmonton, Broc Sawchuk of Wainwright, Sierra Sawchuk of Wainwright; one great granddaughter, Davie Kozak; numerous brothers, sisters, nieces and nephews. He was predeceased by his wife Nancy in 1994; one grandson, Jahryn in 2004.
